Evidence based nursing care is informed by research finding use of research evidence in alnico practice is an expected standard of practice for nurses and healthcare organizations. Because of the large number of patients and the high cost of care associated with hospital readmission nurses both in hospital and in home health service must develop and implement strategies that are associated with improved outcomes for patients.
Healthcare team must conduct a systemic assessment that includes identification of the cost of the CHEF patient’s aggravated factors that may influence the cost of CHEF. In the facility where I work we use evidence-based practice for CHEF patients. CHEF patients are instructed on medication compliance, regular excise, low sodium diet, healthy food and also fluid restriction. Patients were also instructed to weigh themselves daily and report weight gain of 4 pounds in a week. Nurses were also instructed to weigh patients every week and report weight gain of 4 pounds to hygienic.
When a patient weighs more than 4 pounds in a week, action has to be taken; patient has to be evaluated in order to monitor him/her for fluid overload. Patient with CHEF are at risk for fluid overload. After the evidence-based practice, agency noted that the rate of patients readmission were reduced and their conditions were stabilized without frequent recurrence of the disease process.
